I have completed level 40 several times, once with 3 stars, but it always says that the queen has escaped and the next game is at level 40 again. What am I doing wrong?...

What are the requirements to get each of the stars? I can complete a level with no damage and still only get 2 stars. I actually get the exact same score in place of the 3rd star each time....
