To answer your questions:

1. Go in the top door in the middle of the Big Boo room. Press the ? Switch in the Normal Boo room to lower the wall and be able to go in the top door. You'll be at the bottom door in the middle of the Big Boo room. Go right and press the ? Switch to raise a platform from a bottomless pit. Continue right, lure the Big Boo away, press the ? Switch and go in the revealed door.

2. To get to World 5-A, you have to complete TWO secret exits. In World 5-1, after the second Star Coin, there is a high tightrope. Jump to it and you'll see two coins leaving a gap. Jump in the gap to reveal an Invisible Block containing a vine. Climb it to find a circle of Koopa Paratroopas and the last Star Coin. Get past the circle to find a tightrope. Get to the middle and jump to find another Invisible Block containing another vine. After climbing it, moving a little right and falling down, stomp on the Koopa Paratroopas here to the secret exit (this is easier as Raccoon Mario). Completing this secret exit will reveal a Ghost House. This Ghost House's secret exit requires Raccoon Mario. Near the end of the first room, fly up to a small hole in the ceiling where you'll see a door. Go in it and swing on ropes to a ? Switch. Press it to reveal four doors: three being fake. The real door is the one on the top right.
