All you have to do is play single player so many times that you want to kill yourself and you will unlock them. this may take some time, so be warned

U dont exactly unlock it just luck that you get it, it drops better on stage 2

Well you have to win first of all on stage 1 or 2 death matches are best. (winning flawlessly gives you double the chances to get one.)
