Complete list of how to obtain S ranks in all missions ?

I read and followed the one complete list I found on S ranks for this game and most of them were right. However I played mission 12c and had a score over the 17,000 points it said I needed and did not get the S rank. Does anyone have a totally complete and correct S rank achivement list.

Destroy everyone before the mission ends. Fetch the highest possible points within the time limit.
